SSPSK Parental Consent Form
Permission from a parent or guardian, use by use. Nothing is assumed.
Draft for approval by the SSPSK Board.
Please read first
A child cannot give consent to the use of their own information; a parent or guardian must (Data Protection Act, 2019). This form asks for it one use at a time. Tick only what you agree to. A tick for one use is not permission for any other. The coach may not use your child's name, photo or details for anything you have not ticked.
What we collect, and no more
The coach keeps a club register with only: your child's name, your name and phone number, an emergency contact, and a medical note relevant to stacking (for example, an allergy). We do not collect a home address, a child's personal phone number, or health, family or disciplinary information.
How your child is contacted
Coaches contact you, the parent, never your child directly and never through a group chat that includes children.
Your rights
You can withdraw any consent at any time by contacting the SSPSK Safeguarding Focal Point (Gilbert Kipkemoi Kiprop; contact details are being confirmed by SSPSK). Withdrawing consent for photos or data does not affect your child's place in the club. Withdrawing consent for participation ends the place. You can ask for your child's information to be deleted.

This form is completed and printed on your device. It is not sent to or stored by the Safe Stacks website. Give the printed, signed copy to the coach or the school, who keep it with the club register.
Fill this in here, then print it or save it as PDF. Nothing you type is sent to or stored by this website.
I am the parent or guardian of the child named above, and I have read this form. I understand that I can withdraw any consent at any time.
