Used to write the report to you. A nickname is fine.

Your mobile number

How you get back to the report, and where the M-Pesa prompt is sent once the report is written. The last four digits also guard your report link, so a forwarded link on its own shows nothing.

Email (optional)

Only used to send your report link and your receipt.

What class is your child in? (optional)

I am this child's parent or guardian. I agree that their answers and the details above may be stored so this report can be produced and reopened from my link. This is guidance, not a placement, an assessment or a diagnosis. Your report stays open for 150 days. After that your child's answers and the report are erased from our records, so print or save anything you want to keep. We email you 14 days before. I understand the report is written first, and that an M-Pesa prompt for KES 300 will be sent to this number once it is ready — nothing is charged before then.
